Microsoft Access Request Database Template?

trying to create a database that is accessible by a department of people to send requests for different services. I'd like to add a feature that automatically emails the person providing the services with the information the department requests... Does anyone know the name of the function or type of form I need to begin with or maybe a template that performs this way in Microsoft Access 2007?
